Overview

Home care, (also referred to as domiciliary care or social care), is health care or supportive care
provided in the patient’s home by healthcare professionals.

The Health Establishment of Druze Society / Ain Wazein Medical Village, within this context and
contributing to her vision and strategic plan, has established the Home Care Program to offer a
wide range of activities from preventive to end-of-life care, in order to meet the patients’ needs.

The Home care center aims to make it possible for people to remain at home rather than use
residential, long-term, or hospital facilities as well as it encourages and supports the assistance
provided by the family and/or community.

Through the variety of services provided (assisting daily living, nursing interventions,
Laboratory, Physiotherapy, Medical Equipment Services and hospital based services), this center
aims to increase the Quality of Life and the satisfaction of the patients who receive their treatment
and support in their place surrounded by their families, as well as to reduce the number of
admissions and the duration of hospitalization by moving activities to the home of the patient.

Objectives

To help people maintain optimal wellbeing at home by:

  • Determining needs and abilities, developing and coordinating plans of care
  • Teaching self-care and coping skills
  • Improving, maintaining or delaying loss of functional abilities
  • Promoting, teaching and supporting the patients’ families

 

To facilitate appropriate use of health and other community based services by:

  • Preventing or delaying the need for admission to long-term care facilities
  • Supporting people having long-term care admission and offering them Home Care services
    with same approach and safety
  • Preventing the need for hospital non indicated admission, increasing the turnover rate within
    the hospital, and reducing the frequency of re-admission
  • Helping individuals and families to access needed services
  • Promoting primary healthcare services within the community.

The project is divided into five sections respecting the services given and the staff needed. The sections are: Assisted Daily Living (ADL), Nursing Interventions (Procedures), Laboratory services, Physiotherapy services, and Equipment.

Goal
This care is provided to the elderly and the frail people in their family environment while respecting their human dignity and independence to assume both supports, physical andpsychological.
Beneficiaries
  • All patients’ functional dependent (Alzheimer, stroke, fracture, etc…) who want to be within their families and get well in the comfort and security of their own homes.
  • Any patient who can no longer manage to live on their own and doesn’t require complex medical care.
  • Patients admitted in AWMV where their medical condition requires the presence of a companion.

Primary nursing assessment/documentation
  • Measuring vital signs.
  • Documenting vital signs and behavioral change

Goal
  • To provide on-site laboratory specimen collection services to the patients at home, based on the physicians prescriptions.
  • Specimens are delivered to AWMV laboratory center and results will be sent to the patient and physician as soon as they are approved.
Shifts
  • Laboratory daily rounds for blood draw are scheduled in coordination with Ain Wazein Hospital Laboratory Department
  • Appointments, calls answering and results delivery are provided upon request during the hospital working hours.
Beneficiaries
  • All patients who need to do the laboratory tests within a time period of maximum 3 hours between collection and arrival of specimens to AWMV Laboratory.
  • All laboratory tests could be done except the ABG's, ammonia, lactic acid, urea breathe test and semen analysis.
  • Post-surgical care
Goal
To provide patients with a professional and safe setting that allow them to avoid hospitalization and gain nursing services that enhance their health condition.
Beneficiaries
  • Patients who need nursing services away from the hospital
  • Patients who have medical prescription and who are in need of Nursing intervention and/or patients post discharge from AWMV.

Schedule will be adjustable upon needs, out of AWMV working hours, and before 08:00 pm.

Hours of Service

  • Planned, intermittent, and predictable service is available to patients within the Chouf area at any time (e.g. weekends, evenings, nights, and holidays).
  • Coverage is available 24-hours a day, 7 days a week through an answering service.
  • Direct phone lines (landline # 7044 and mobile hotline 03707047) are available in the Home Care Center office to allow immediate access of the patient to the services.
